Prerequisite Courses

All prerequisites must be completed before applying to the Occupational Therapy Assistant program, or if currently enrolled in a prerequisite course, official grades must be posted to your SLCC Degree Works transcript before the application deadline. However, if you are waiting for transcript grades you can still submit your application materials and then add the grades as they are available prior to the deadline.

Note: BIOL 2320, BIOL 2325, and OTA 1020 must be completed with a grade of B or better within the past five years; limited to two attempts per course. MATH 1010, 1040 or higher must be completed with a grade of C or better within the past five years. The remaining prerequisites must be completed with a C grade or better.

HESI Health Sciences Pre-Admission Exam

Tests must be scheduled at least one week in advance.
You must pass with a minimum benchmark score of 75%.
To raise your score, you may retake the exam after a two week remediation period. You are allowed to take the HESI a total of two times every two years.
HESI scores will be valid for two years.

Schedule HESI Online Tutoring is NOW ACTIVE!!! Online Tutoring will offer Online Tutoring in more subjects than ever before. Students will have a single sign on thru their Canvas courses (on left hand navigation column).

Highlights: Students can request a Tutor in their native language. Student’s sessions with a Tutor are archived so the student can go back and review a previous session over and over. There is also a Skills Lab, Study Center, Flashcard maker, and Practice tests for the GED, GRE, Accuplacer, HESI, and TEAS.

Community Service & Letter of Recommendation

Complete 25 hours of community service. Your community service should not be Occupational Therapy related. Fill out the Community Service Verification Form and submit with service confirmation letter and reflection. Send your form to

Background Check and Compliance Package

To order Criminal Background Check and Compliance Packages:

  1. Go to Complio - background check and compliance system.
  2. Follow the instructions to set up an account.
  3. Order the Criminal Background Check Package ($29.00) and the Compliance Package ($15.00). The compliance package is a 12 month subscription that will need to be renewed every 12 months while you are in the program at a cost of $15.

Do not order the Drug Screening Package ($35) at this time. We will notify you at a later date to purchase a Drug Screening Package. If you go ahead and order it at this time, SLCC will not reimburse you and you will have to order and pay for it again at a later date.

The Background Check, Compliance and Drug Screening must be completed before you begin your internship.

You assume the risk if your criminal background check is not clear. If you do not pass the background check, you may not be accepted into the program, which may result in withdrawal from the program after the tuition refund deadline has passed.

It is strongly recommended that prospective OTA students take measures to have criminal offenses (felonies and or misdemeanors) expunged or sealed from your record if possible.


Prior to beginning your first semester, you must provide proof of the following immunizations:

  1. Annual Influenza Vaccination:
    Generally given September-February. You must provide proof of the influenza vaccination or submit a signed declination. This is not required for admission, but will be required once you are in the program. (You may be required by a clinical site to have the influenza vaccination)
  2. Tuberculosis Blood Test: Required Annually
    Proof of a negative QuantiFERON TB Gold test or T-SPOT.TB; if positive, you must submit a chest x-ray read by a radiologist with a normal result; if the chest x-ray is abnormal, you must be cleared by your physician or local health department. This is due by May 15th. This requirement also includes international students who have had the BCG immunization. Once in the program an annual Quantiferon is due by the date shown in Complio. The need for a repeat chest x-ray will be done on a case-by-case basis.
  3. Tetanus, Diphtheria and Pertussis (Tdap) Booster:
    Tdap booster is good for 8 years. TD will be given subsequent to Tdap. This is due in Complio by the application deadline of May 15th.
  4. Measles, Mumps, Rubella (MMR):
    Proof of two doses or a positive titer (blood draw) on all three (measles, mumps and rubella). Equivocal results are not acceptable; you must test in the positive range if you choose to do a titer. This is due in Complio by the application deadline of May 15th for admission. NOTE: If given to adult females, since this is a live virus, you cannot be pregnant at the time you receive the immunization and you must avoid pregnancy for 3 months after administration
  5. Varicella (Chicken Pox):
    Proof of two doses or a positive titer (blood draw). Stating you had the disease is not acceptable. Varicella immunizations are expensive, if you had the disease it is recommended to start with a less expensive titer first for proof of immunity before starting the immunizations. This is due in Complio by the application deadline of May 15th for admission. Once in the program if titer was negative, student must submit 2 doses of the vaccine taken 30 days apart and 30 days after the negative titer. Documentation of the two doses is due in Complio by August 1st.
  6. Hepatitis B vaccination series:
    You must submit a titer (positive or negative) by the application deadline of May 15th for admission. Once in the program, if the titer was negative or equivocal – 1) Submit the dates of your 3 dose series of shots in Complio, 2) Reinitiate the immunization series immediately – repeat shot #1 due by July 1st, shot #2 due by August 1st, shot #3 due by March 1st, and repeat titer due by May 1st.
  7. Hepatitis A vaccination series: Highly recommended but not required
    Proof of a two dose series or a positive titer. If you start the series, you must complete it.

The SLCC Center for Health and Counseling offers immunizations at a discounted rate for students who are low income. For a discounted rate, take your previous income tax return or your last 30 days pay stubs.

Some clinical facilities may require additional immunizations.

CPR Certification

Only American Heart Association Two-Year BLS Providers will be accepted. Your CPR certification must be kept current throughout the program. This is not required for admission, but will be completed once you are in the program by September 15th.

BLS Provider

The new BLS Course replaces the BLS for Healthcare Providers (BLS HCP) and BLS for Prehospital Providers (BLS PHP) courses. The BLS Course teaches both single-rescuer and team basic life support skills for application in both in- and out-of-hospital settings. This course trains participants to promptly recognize several life-threatening emergencies, give high-quality chest compressions, deliver appropriate ventilations, and provide early use of an AED. It includes adult, child, and infant rescue techniques. Provider cards are available exclusively to authorized TCs for issuance in accordance with the AKA policy.

BLS Healthcare Provider

The BLS for Healthcare providers and BLS for Prehospital Providers Courses have been replaced by the 2016 BLS Course.

  • Not valid for issue after April 16, 2016, and is superseded by card 15-1805

Health Sciences Program Admissions

Admission information for SLCC Health Sciences programs is available on each program’s Admissions page listed in the table below. The Health Science programs use either a selective or a rolling admissions process, separate from the general college admission process. The cost for each application is $25.00.

Students who have applied more than once in a year can be refunded the second application fee by emailing This policy does not apply to students who are accepted to and begin a program but do not successfully complete it.

The Disability Resource Center provides reasonable accommodations to students, but you’re responsible to disclose your disability and to provide appropriate documentation to receive accommodations.

You’re also responsible to meet reasonable expectations in your courses, and to seek the services you may need from other departments, such as Academic Advising, the Learning Center, the library, etc.

In addition, you should:

  • Deliver Accommodation Authorization Letters to your instructors.
  • Renew accommodations each semester after registering for classes, and discuss any changes needed to your accommodations.
  • Contact your Accessibility Advisor if changes occur, or if you experience problems, or if your accommodations are interrupted.

One of the biggest hurdles for applicants is getting their transcripts evaluated in time. Please make sure that your transcripts are submitted at least 4 weeks before the application deadline (8 weeks for out of state transcripts) or there is a possibility that they will not be evaluated before the application closes. This could put your application in jeopardy and cause you to have to reapply the next time the application is open.

If you are accepted to a program a confirmation email will be required, please read all emails carefully and thoroughly to make sure you don't miss any information.

If you are interested in enrolling in the Pathophysiology (NURS 1130) course before you have been accepted to the Nursing program, or want to take it because you are applying to another nursing program you will need to complete all the prerequisites with a grade of C or better. Prerequisites cannot be in progress but must be complete before enrolling in this course.
